Case Management:
Facilitation, care coordination and advocacy for services to meet the individual and family’s needs.
Mentoring:
Offering guidance and encouragement leading an individual to success.
Activities of daily living:
Daily self-care tasks such as, personal hygiene and grooming, scheduling appointments, apartment/home maintenance, taking medication as prescribed, managing money, paying bills, using technology, shopping, meal planning and food preparation.
Community Living Skills:
Assessing community resources including, transportation (public and/or private), banking, library, social groups or clubs, and shopping.
Job Development/Job Coaching:
Assistance with job search, applications and interviews. Once a job or volunteer work has been obtained, job coaching as necessary,
Entitlements:
Services or financial assistance provided by state or federal government that individuals with disabilities can apply for. These include, but are not limited to ACCES-VR (adult career and continuing education services/vocational rehabilitation, formerly VESID), Social Security/Disability (SSI/SSA), Food Stamps/SNAP benefits, Subsidized Section 8 Housing, Medicaid or Medicare.
Professional Referrals:
To psychiatrists, psychologists, social workers, medical professionals, and day programs.
Spanish/English bilingual:
Arturo speaks fluent Spanish and can provide services to Spanish speaking individuals.
